Disease stage
Required: Stage III, IV
Performance status
ECOG 0–2(Ambulatory, capable of self-care)

Blood counts
neutrophils ≥ 1.5×10^9/l, lymphocytes ≥ 0.8×10^9/l, hemoglobin ≥ 100 g/l, and platelets ≥ 75 × 10^9/l
Kidney function
creatinine clearance ≥ lln (cockcroft-gault formula)
Liver function
total bilirubin ≤ 2×uln, alanine aminotransferase ≤ 3×uln, aspartate aminotransferase ≤ 3×uln
